Medication Management Service

Developed for Canterbury Clinical Network and now being implemented through collaboration between CDHB, CCPG and Pegasus. MMS service including the provision of a mobile MMS Pharmacist services to compliment and support the development of Community MMS Pharmacist services across the Canterbury region.

The service utilizes a multidisciplinary model amongst health providers to support better health outcomes for patients. This service is expected to deliver a significant reduction in medicine oversight requirements, a release of resource to support older patients in the community reducing residential care requirements and have a positive impact on presentations to ED and AMAU.

Definition of Service: Medicines Use Review (MUR) is a structured, systematic, documented and consultation-based service undertaken by an accredited pharmacist. MUR aims to improve the patient’s understanding of their medicines-related health outcomes by identifying access, adherence, and day to day management issues a patient has with their medicines and setting goals with the patient to resolve these issues.

To reduce the potential for confusion that Medicine Use Review involves a clinical review of medication, Canterbury wishes to use the name “Medication Management Service” (MMS) which will be provided by MUR accredited pharmacists.

Medicine Use Review (See Locum MUR Proposal) process for Canterbury including a mobile MUR pharmacist. As a part Medicine Management Work stream within the Aged Care work stream of the CCN. Medicine review as part of rationalising nursing visits.

By the end of October 2011 the service will have a complement of four mobile pharmacists (3.0FTE) and as at 1 October over half of all Canterbury pharmacies have accepted the CDHB’s offer to deliver MMS.
